Community Health Assessment Project (CHAP) Final Paper (Paper Grading Rubric for Written Assignment) I. Description The purpose of this course is concerned with the health and well-being of vulnerable populations as aggregates within the community. The final project will be a field based experience for the student. The intent of this project is to link course content to practice. An integrative approach along with the nursing process will be used to complete a community assessment of a selected communitys health, wellness needs, and available resources (Stanhope & Lancaster, 2012, p. 406). A minimum of three research articles from the scholarly literature is required. III. Instructions for Completion of Assignment and Grading Scheme (Total 300 points) A. Introduction (20 Points) o Your introduction should contain a thesis statement. This statement should explain the theme for your paper; succinctly stating purpose and conclusion. Also in your opening paragraph(s) you will want to set the stage or provide succinct information about your main idea; explaining what this paper is important o Remember your goal is to engage your reader B. An Integrative Approach to Community Assessment (50 points) o Describe how the integrative approach and nursing process will be combined to guide the community health assessment. o Support why a community health assessment is important to the community and the role of the community nurse as participant, promoter, or coordinator of the event. C. Primary and Secondary Data (80 points) o Introduce the selected geographic area for the community based assessment including the zip code. o Provide a written descriptive assessment of the community; defining the client (community) in terms of place, people, and function (See Stanhope & Lancaster, 2012, p. 406-410). o Attach the Windshield Survey as Appendix A (using APA format) o Describe the relevance of the nursing assessment as it relates to primary and secondary data relevant to the community assessment including the three levels of practice (population, systems, and individual/family). o Attach Secondary Data Collection Tool as Appendix B (using APA format) after the reference sheet. o Develop a composite database. D. Social Determents of Health (SDOH) (40 points) o Interpret the composite database o Identify community problems and strengths; problem analysis (See Stanhope & Lancaster, 2012, p. 411). o Analyze a selected community problem o Identify the factors contributing to the problem and explain the relationship to the problem o Consider the data trends and how your population compares to what is known at the county, state, or national level. o Attach the Problem Identification and Analysis Worksheet as Appendix C (using APA format) after the reference sheet. E. Establishing Goals and Objectives (40 points) o Prioritize the problem (See Stanhope & Lancaster, 2012, p. 413-417). o F. Implementation and Evaluation (25 points) o Propose one or more implementation mechanisms or modes by which the change would be implemented and evaluated G. Conclusion (25 points) o Provide a strong conclusion the goal is to sum up what you have written. No new information should be provided. Consider that you want to restate your main idea, summarize your major points and leave the reader with a good final impression.
