Adequate literature reviews on fall assessments and prevention interventions are used in this study. Consistent evidence on standardized interventions on fall prevention among hospitalized patients is lacking. The ACW KBNI provides a useful framework to assist nurses in piloting an inclusive fall-risk assessment and modifying prevention interventions to decrease the risk for a fall. A randomized control trial by Ang, Mordiffi, & Wong (2011) reveals the productiveness of targeted multiple intervention strategies according to the fall-risk assessment in decreasing the number of falls in high-risk patients. This study was led in Singapore at an acute care hospital in 2006. The study result shows that the use of a valid and reliable fall-risk assessment tool had enabled the nurses to give customized care to each high risk patient and the multiple intervention strategies according to the risk assessment along with usual care that does a better job in decreasing the number of falls in hospitalized patients. A randomized study by Dykes, et.al (2010) was conducted in four urban hospitals in the US to compare the hospital fall rate and to investigate Fall Prevention Tool Kit (FPTK) which reduces the patient falls in hospitals. Based on fall-risk assessments, the nurse tailored the FPTK to address patients specific factors of fall-risk. The study result shows that the use of Fall Prevention Tool Kit is based on fall-risk assessments that significantly reduced the fall rate in hospitals. Another study by Kolb (2004), was to find out the effects of the introduction of fall-risk assessments and fall prevention protocols in the incidence of a fall and to investigate the accuracy of the Morse Falls Scale. The teaching and research hospital in Western Victoria, Australia was targeted for this study. Quasi random sample of two cohorts were used and multitargeted strategies were implemented. The study results highlight that a decrease in fall rate might be attained by precise evaluation and multitargeted interventions from admission to discharge. The Morse Scale was easily adopted to different contexts. AHRQ (2013) established a tool kit to prevent falls and fall related injuries based on systematic reviews of current literature. In this toolkit, the assessment of risk factors by an assessment tool is a standardized process in planning care for patients. Patients are receiving individualized care according to the fall risk score. Incidents of falls are the leading cause of morbidity and mortality in older patients and hospitalization increases the risk. The studies reveals that fall risk assessments are tailored with fall prevention interventions which will reduce the incidents of falls in hospitalized older patients.
